Candidates can fill the NCHMCT JEE 2021 application form through online mode. The application form has been released from 3rd February 2021. Application form will continue till 10th May 2021. Candidate are advised to fill application form carefully and before last date of submission. Incomplete or late submission of application forms can cause disqualification of candidate.
Candidate should fill the application form carefully. Incomplete & multiple application forms can get rejected. Candidates have to upload their scanned photograph & signature in the application form as per the prescribed format. No request for change/correction in the particulars of application form will be allowed later on.

How to fill NCHMCT JEE 2021 Application form
Candidates can fill the application form of NCHMCT JEE 2021 by following the steps given below:
Log on to the Website:
- Candidates in the first step have to visit the website of NCHMCT (Link is given above).
- Now, log in by providing the details like Name, Category, DOB and Email id.
- After submitting the details, an SMS will be sent to the candidates email id about the confirmation of successful log in.
Filling the Application Form:
- Now, candidates have to again log in using email id and DOB.
- Fill the application form by providing the information of candidate’s name, father’s name, mother’s name, gender, date of birth, exam city, category, address, educational qualifications, email id etc.
Upload Signature and Photograph
- Candidates in the next step are required to upload their scanned signature and photograph as per the below specifications:

Payment of Application Fee
- Candidates after the successful uploading of the images, have to pay the application fee for NCHMCT JEE 2021.

- After the successful payment of application fee, candidate can now save the application form.
- After that take the print out of the submitted application form.
- Candidates are advised to keep the print out of the application form for further use.
NCHMCT 2021 Eligibility Criteria
Candidates are advised to insure their eligibility before applying for exam. Here we have mentioned complete eligibility criteria to be eligible for NCHMCT 2021 Exam:
- Qualifications: Candidates should have pass 10+2 intermediate or equivalent exam form any recognized board.
- Stream: Candidates can have any stream with English as mandatory subject.
- Age Limit: There is no lower age limit for examination but applicant should not be more the 25 year old. (3 relaxation will be allotted to the reserve category candidates)
- Appearing: candidates appearing in final examination of 10+2 intermediate or equivalent can also apply for examination.
- Medical Certificate: Candidates must be physically fit to be eligible for Examination.
